Renewal Affirmations: Please check the lines that apply and attach the appropriate supporting documentation:
I have applied within my designated two-year certification period. Please note: there is a 30-calendar day grace period for late renewals past the certification period. Late submission of renewal applications must be postmarked within this 30-calendar day grace period. Renewal during this grace period will result in a $25 late fee.
The grace period does not provide additional time to earn required CEUs.
I have a current state-issued social work license to practice at the appropriate level of my current position and I have attached a copy of my license showing its expiration date.
My state-issued social work license has not been revoked or suspended in the last 2 years and I have no pending, professionally-related legal or ethical complaints.
I have earned a minimum of 20 hours of CEUs with specific content and learning objectives in Oncology, Palliative Care and/or End of Life Care, all of which were earned during my two-year certification period.
Please check the statement below which describes your current social work position:
My direct/clinical social work practice is full-time (defined as 32-40 hours/week) with a minimum of a 50% or more of my workload specifically in Oncology Social Work.
My direct/clinical social work practice is part-time (defined as 20-31 hours/week) with 100% of my workload specifically in Oncology Social Work.
My non-direct/non-clinical position is a full-time position (defined as 32-40 hours/week) with a minimum of at least 50% of my workload in oncology specific program development, clinical supervision, research and/or teaching.
Oncology social work loads of less than 20 paid hours per week do not meet the criteria for OSW-C.
OSW-C CEU Renewal Criteria:
Proof of a minimum of 20 hours of CEUs must be earned and submitted within each two-year renewal cycle. The CEUs must have specific oncology, palliative care or end of life care content and learning objectives. Please see on-line renewal application for further specific details regarding qualified CEUs. CEUs can be obtained from, but not limited to, AOSW, ACS, APOSW, APOS, Cancer Care, ACCC, NHPCO, NASW, and ONS. CEUs may be earned in person or online. Please see FAQ section for information about CEU’s for renewal.
CEUs must be earned during the two years of your renewal cycle. For example, if your OSW-C expires on 5/31/18, you must earn CEUs between 5/31/16 – 5/31/18 for your renewal.
General CEU topics regarding depression and anxiety do not count for this certification requirement unless taught for specific oncology population.
CEU certificates from these programs must be in English and enclosed or attached with the renewal application. They are the only acceptable documentation to verify required CEU criteria.
Annual hospital safety programs, Tumor Boards, journal clubs, care conferences, staff meetings, team meetings, Rounds (including Schwartz Rounds) and/or Cancer Committee meetings do not count towards CEU requirements,
social work CEUs have been approved for the presentation.
CEUs are obtained in the following ways:

Nursing and AMA oncology focused CEUs may be considered for acceptance if

content and learning objectives related specifically to psychosocial oncology, palliative care or end of life care.
Teach a social work course in a CSWE-approved school of social work which has oncology, palliative care or end of life focus. For each credit hour of the class, two CEUs will be awarded. For example, a three-credit course would be awarded six CEUs.
Publish an article in a peer-reviewed professional journal or a chapter in a peer-reviewed professional textbook relevant to psychosocial oncology, palliative care and/or end of life care.
Publications outside of the specific renewal period cannot receive credit in the current renewal period.
For each publication, five CEUs can be awarded. A maximum of 10 CEUs in any two-year renewal period can be awarded for publications.
Present a newly developed social work CEU-approved presentation specifically focused on psychosocial oncology, palliative care and/or end of life care. For every approved social work CEU awarded to the attendees, the presenter will be credited double CEU credit. For example, a presentation offering 3 approved social work CEUs would result in 6 CEUs for the presenter.
This presentation will qualify for CEUs only for the first time it is presented, and the presentation must occur during the two-year certification period.
Public speaking, community presentations and any other lectures which do not provide approved social work CEUs
meet OSW-C continuing education criteria. These activities can be counted toward required community/leadership activities.
